The monstrosities in the SRD
The 39 monstrosities here span CR 1/2 to CR 30, so the type covers several tiers of play — 21 large-sized and 9 medium-sized, with 9 more across other groups. Size matters more than the type label in practice, because it decides reach, grappling and how many of them fit in a corridor. Sort by challenge rating when you are building an encounter, and by size when you are working out whether the fight fits the room.
Running monstrosities
Monstrosities are the catch-all for creatures that are unnatural but not supernatural, and their value is that players cannot predict them. Nobody knows what an owlbear does until it does it. Most have one distinctive mechanic — a grapple, a gaze, a petrification — and the fight should be built around the party discovering and answering it.
What they need from you
Because the defining trait is usually a single ability, telegraph it. A statue-strewn cave before the basilisk, bones outside the lair, a warning from a survivor. Players who walk into a save-or-die effect with no warning feel cheated; players who spotted the signs and prepared feel excellent. The mechanic is the same in both cases — only the foreshadowing differs.
